Booking an appointment is easy.
- If you are interested in a vasectomy reversal, you can self-refer by completing the form below. Consultation fee is $195.
- If you are interested in a sperm retrieval please have your family doctor complete a referral to our clinic to initiate this process. If you do not have a family doctor, a walk-in clinic can provide a referral.
